Especially for independent practices and the people they serve.</p></div></div></div></div></div><div class="thrv_wrapper thrv-columns" style="--tcb-col-el-width: 738.812;" data-css="tve-u-685076e2597219"><div class="tcb-flex-row v-2 tcb--cols--1" data-css="tve-u-685076e2597200" style=""><div class="tcb-flex-col" data-css="tve-u-685076e2597221" style=""><div class="tcb-col"><div class="thrv_wrapper thrv_text_element" data-css="tve-u-685076e25971f7"><h3 class=""><strong><strong>We Understand the Real Reasons Value-Based Care Keeps Failing<br></strong></strong></h3></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">1. Misaligned Incentives</span></p><p dir="ltr">Even as value-based care expands, most physicians are still paid through fee-for-service. Their <a class="" href="https://www.kff.org/medicare/issue-brief/what-to-know-about-how-medicare-pays-physicians/" style="outline: none;" target="_blank">income depends on volume</a>, not outcomes. That leaves little reason to invest time in prevention, coordination, or long-term engagement.</p><p dir="ltr">Even when contracts include shared savings, those dollars rarely reach the people delivering care. Administrative middlemen—<a href="https://vbctransformationpartners.com/analysis-of-payor-contract/" target="_blank">payers</a>, systems, or MSOs—often absorb the gains. That disconnect erodes trust and reinforces a common belief: <a href="https://vbctransformationpartners.com/value-based-care-complexity/" target="_blank">value-based care</a> benefits everyone but the provider.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">2. Administrative Complexity</span></p><p dir="ltr"><a href="https://vbctransformationpartners.com/what-small-practices-can-teach-the-industry-about-value-based-care-if-we-listen/" target="_blank">Independent practices</a> run lean. Most don’t have compliance teams or analysts—but <a href="https://vbctransformationpartners.com/value-based-care-complexity/" target="_blank" class="" style="outline: none;">value-based care programs</a> still expect them to meet complex reporting requirements layered on top of fee-for-service.<br><br>Clinicians now juggle billing, quality metrics, and duplicative documentation across systems that don’t align (<a class="" href="https://www.fastcompany.com/91344000/value-based-care-how-good-ideas-get-crushed" style="outline: none;" target="_blank">Newman, 2025</a>). Instead of streamlining care, value-based care often adds bureaucracy.<br><br>This burden is massive: administrative waste accounts for 7.5%–15% of all U.S. healthcare spending—up to $570 billion a year (<a href="https://www.fastcompany.com/91344000/value-based-care-how-good-ideas-get-crushed" target="_blank" class="" style="outline: none;">Newman, 2025</a>). That’s money spent on paperwork, not patients.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">3. Weak Data Infrastructure</span></p><p dir="ltr">Value-based care <a class="" href="https://vbctransformationpartners.com/real-time-data/" style="outline: none;" target="_blank">depends on good data</a>—but most independent practices can’t access it. EMRs are fragmented, billing systems lack nuance, and few practices have the tools to make sense of it all (<a href="https://www.healthaffairs.org/content/forefront/rescuing-value-based-healthcare-through-better-data" target="_blank">Hobson et al., 2025</a>).<br><br>Even large employers struggle to get clear insights into cost and quality. If payers and providers can’t see value, they can’t reward it.<br><br>Worse, many <a class="" href="https://vbctransformationpartners.com/value-based-care-complexity/" style="outline: none;" target="_blank">value-based care metrics</a> come from insurers, not clinicians. That leaves providers evaluated by standards they didn’t help define, using tools they can’t access. Without visibility, there’s no clear path to improve—or get paid for it.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">4. Risk Without a Net</span></p><p dir="ltr">Independent practices are right to be cautious about downside risk. Many operate on tight margins. Taking on financial exposure without support feels like walking a high wire without a net.</p><p dir="ltr">Just <a href="https://www.healthaffairs.org/content/briefs/value-based-payment-tool-address-excess-us-health-spending" target="_blank" class="" style="outline: none;">6.7% of U.S. healthcare payments are fully capitated</a>. Most still rely on fee-for-service, with modest bonuses—far from true financial flexibility.</p><p dir="ltr">Even two-sided risk models, which offer potential bonuses, can backfire. Small practices often lack the reserves, infrastructure, or protections to succeed under pressure. Without stepwise support and tailored design, <a href="https://vbctransformationpartners.com/value-based-care-complexity/" target="_blank" class="" style="outline: none;">value-based care</a> feels like a liability, not an opportunity.</p></div><div class="thrv_wrapper thrv_text_element" data-css="tve-u-685076e25971f7"><h3 class=""><strong>There’s a Way Forward—Especially for Independent Groups<br></strong></h3></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr">System-wide transformation may be slow, but small and mid-sized practices aren’t stuck. In fact, their size and autonomy give them the agility to do value-based care differently—and more effectively. Here’s how.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">1. Focused, Flexible Roadmaps</span></p><p data-css="tve-u-1977a8474c1" dir="ltr" style="">Most value-based care models are built for large systems and rolled out as top-down mandates. <a class="" href="https://vbctransformationpartners.com/what-small-practices-can-teach-the-industry-about-value-based-care-if-we-listen/" style="outline: none;" target="_blank">Independent practices</a> need the opposite: <strong><em>flexibility</em>.</strong></p><p dir="ltr">Start small. Phase in. Align each step with how you already work. That reduces disruption and increases relevance.</p><p dir="ltr">A thoughtful roadmap doesn’t ask, “What should we do?”—it asks, “What are we ready for, and what will move the needle right now?”</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">2. Redesign Workflows for People, Not Just Metrics</span></p><p data-css="tve-u-1977a8474c1" dir="ltr" style="">Burnout is real. Post-COVID, clinicians face more work, fewer staff, and rising pressure. Any model that adds tasks without relief is set to fail.<br><br>Redesign for the people delivering care. That means <a href="https://vbctransformationpartners.com/clinical-documentation-integrity-and-quality-management-model/" target="_blank" class="" style="outline: none;">streamlining workflows, building team-based support, and using tools that work for clinicians</a>—not just auditors.<br>Metrics matter—but they should grow out of good care, not dictate it.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">3. Embrace Data Differently</span></p><p "="" class=" data-css=" dir="ltr" style="" tve-u-1977a8474c1"="">More dashboards don’t mean better insight. Most practices are buried in data that’s too fragmented to use.<br><br>What small groups need is clarity: <a class="" href="https://www.healthaffairs.org/content/forefront/rescuing-value-based-healthcare-through-better-data" style="outline: none;" target="_blank">clean, usable insights tied</a> to outcomes they actually influence. Decision-ready data. Simplified visuals. Actionable benchmarks. With the right tools, <a class="" href="https://vbctransformationpartners.com/how-vbc-transformation-partners-can-help/" style="outline: none;" target="_blank">data becomes a strategic asset</a>—not a reporting burden.</p></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr"><span style="text-decoration: underline;">4. Build Stronger Employer-Provider Alignment</span></p><p dir="ltr">You don’t have to do this alone. Employers—especially self-funded ones—are looking for care partners who can deliver real <a href="https://vbctransformationpartners.com/value-of-investment-voi/" target="_blank" class="" style="outline: none;">value of investment</a>.</p><p dir="ltr">Many are already pursuing reform: 85% are expected to use Centers of Excellence (COEs), 46% will be engaging High-performance Networks (HPN), and nearly a third will have implemented value-based primary care in 2025 (<a href="https://www.businessgrouphealth.org/en/resources/2025-employer-health-care-strategy-survey-intro" target="_blank">Business Group on Health, 2024</a>). These trends show a clear appetite for delivery models that prioritize quality, efficiency, and direct alignment.</p><p dir="ltr">This is a chance for independent groups to bypass the middlemen by forging direct contracts, joining high-performance networks, and building value-based specialty partnerships that truly reward good care.</p></div><div class="thrv_wrapper thrv_text_element" data-css="tve-u-685076e25971f7"><h3 class=""><strong>This Isn’t Just a Systems Problem. It’s a Design Problem.<br></strong></h3></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr">Value-based care didn’t fail because the idea was flawed. It failed because the system never let it live. Instead of replacing outdated models, we stacked new ones on top—until clinicians were buried in dashboards, data entry, and duplicative expectations.</p></div><div class="thrv_wrapper thrv_text_element" data-css="tve-u-685076e25971f7"><h3 class="" style="color: var(--tcb-skin-color-0) !important; --tcb-applied-color: var$(--tcb-skin-color-0) !important;"><strong><em>The system didn’t kill value-based care. It just buried it in paperwork.</em><br></strong></h3></div><div class="thrv_wrapper thrv_text_element"><p dir="ltr">That’s not the end of the story. Because when you strip away the bureaucracy, the core principles of value-based care still resonate. Coordinated care. Preventive action. Measurable outcomes. Health equity. These aren’t policy buzzwords—they’re the building blocks of better medicine.</p><p dir="ltr">Independent practices have a unique opportunity to reclaim that vision. They’re not burdened by scale. They’re not locked into decades of structural inertia. With the right tools, partnerships, and cultural redesign, they don’t have to wait for permission.</p><p dir="ltr">They can lead. Right now.</p><p dir="ltr">This isn’t just a systems problem. It’s a design problem. Design is something <a href="https://vbctransformationpartners.com/what-small-practices-can-teach-the-industry-about-value-based-care-if-we-listen/" target="_blank">independent practices</a> are still agile enough to change—on their own terms, for their own people, and for the future of care itself.</p></div><div class="thrv_wrapper thrv_text_element" data-css="tve-u-685076e25971f7"><h3 class=""><strong>Let’s talk about what that could look like for you.<br></strong></h3></div><div class="thrv_wrapper thrv-button thrv-button-v2 tcb-local-vars-root" data-css="tve-u-685076e2597232">
